Closing the Gap programs help Aboriginal and Torres Strait Islander people who have a chronic disease or are at risk of developing a chronic disease. You can sign up for Closing the Gap programs with your Regular Doctor.
There are two programs that Aboriginal and Torres Strait Islander people can sign up for:
How do you sign up with our practice?
Do you identify as Aboriginal and or Torres Strait Islander? Speak with the Receptionist to book your yearly Aboriginal Health Check. Once you have completed this health check with our Practice Nurse an appointment with your Regular Doctor will be made. During this appointment the Doctor will complete all necessary forms for you to register and explain the services and benefits you are entitled to.
You will need to register each calendar year to receive the services and benefits. In January you will receive a phone call from our practice to arrange the necessary appointments to ensure you remain registered in the program.

There are two programs that Aboriginal and Torres Strait Islander people can sign up for:
- Closing the Gap Pharmaceutical Benefits (PBS) Co-payment measure: Provides low-cost or free medications for Aboriginal and Torres Strait Islander people who have a chronic disease or are at risk of getting a chronic disease.
- Practice Incentive Programs – Indigenous Health Incentive: Provides help for Aboriginal and Torres Strait Islander people aged 15 years and older with chronic diseases. Chronic diseases include asthma, cancer, cardiovascular illness, diabetes, musculoskeletal conditions, stroke and other conditions. To sign up you will need a Medicare card. The Program covers:
- Health Checks
- Visits with eligible allied health services and or visits with a practice nurse
- Eye checks
