Welcome!
We welcome new patients to St Agnes and Tea Tree Surgery. If you are a new patient and have not visited either practice please be advised of the following:
- Please arrive 10 minutes early for your first appointment as we will need you to complete several forms prior to seeing the Doctor. Please print off the New Patient Form below and fill it in prior to arriving. If you are unable to print we will have copies available at Reception.
- Bring your valid Medicare, Government Concession and/or DVA cards with you.
- If you would like a summary of your Medical Records from your previous practice please speak with Reception and we can assist with the transfer from your previous Doctor.
Should you need to change or cancel your appointment please ring 2 hours prior to your appointment.
Please review our Fees for Consultations and Surgical Procedures. All invoices are to be paid on the day. Please bring a form of payment (Cash, Eftpos or Credit Card payment are available)

If you are a new patient at our practice, please feel free to print off, fill out and bring your registration form to your appointment.
Alternatively, on arrival to your first appointment, you will receive a New Patient Pack, where these forms are included.
