Workers with work related injuries will need to inform your employer prior to the consultation and bring your employer’s contact details for verification.
Please fill in the Return to Work SA form (see link below) prior to arriving for your appointment.
Please note that all invoices are to be paid on the day of your consultation. It is the patient's responsibility to obtain reimbursement from your employer or Return to Work South Australia.
If your claim is denied, the consultation is not reduced but you will be able to submit your claim to Medicare for part reimbursement.
