Covid 19 Booking Information

Covid 19 Booking information
  • The *​AstraZeneca  Covid 19 Vaccination will not be available at  our Practice after 25/12/21
  • *Pfizer vaccination is now available at our Practice for all patients 5yrs old 
  • *Children 5-11ys will have their own special Pfizer immunisation clinic*  *Please request this clinic when booking.*
  • *Booster Vaccinations are now available -  Please see the "Covid 19 Page" for criteria
  • All Covid 19 Vaccination Clinics will take place at St Agnes Medical Centre.
  • As we receive supplies of vaccinations appointments are opened.

​We thank all our patients for your patience in making appointments.  With a patient base of over 50,000 patients we appreciate that it has been difficult to not only make on line bookings but get through on the phone.
​
**Please be patient and understanding when we are unable to offer appointments. 
​We are receiving hundreds of phone calls per day with limited vaccines being supplied by the government

How Do I make a Booking
Online Booking - Covid Vaccination
*As of 15/9/21 only Pfizer vaccinations will be available through online bookings.
​Please ring the Surgery if you require an AstraZeneca vaccination
*
You will then be directed to our booking Website. Click on the "COVID Immunisation Clinic" tab to make a booking for a Covid Vaccination.  ​
       Choose the correct Vaccination Clinic - Either   1) 12years and over
​                                                                                    OR   2)  5-11years
**Note that you can NOT book for Covid Vaccination with individual Doctors. Vaccinations can only be made through the COVID IMMUNISATION CLINIC**

  • Pfizer Vaccination is now being given to all patients 5yrs and older.  
                                Please see the "Covid 19 Page" 
  • ​Appointments can be made:   1) Online  - Click the link  above​
                                                               or 2) Call our Surgery
  • Two appointments  3 weeks apart will be required for Pfizer if you are 12yrs and older.
  • For children 5-11yrs the gap between the two primary vaccinations is 8 weeks.
  • A a 3rd primary dose of COVID-19 vaccine is now recommended in severely immunocompromised populations. This is given 2-6 months after the 2nd injection. Pfizer or Moderna is preferred to AstraZeneca for this 3rd dose.   (Further information link)
  • A covid booster vaccination is now also available. This is given 3 months after your last covid vaccination. Pfizer or Moderna are the recommended vaccinations.  Only Pfizer will be available at our Surgery 
  • A 2nd covid Booster vaccination is available from April 2022. It is given 4 months after the 1st booster. See the "Covid 19 Page" for criteria.

​Prior to your appointment obtain a copy of the St Agnes Surgery Covid Immunisation "Check List", either from our Surgery or download it here. Bring it with you to your vaccination appointment along with the requested information.

  • Your medical history will be checked
  • Your "Check List" will be checked
  • You will be required to sign a consent form:     
                                  Download the consent forms, fill them in and bring them in to save time. 
                                  Consent form for 1st Covid Vaccination        OR        Consent form for 2nd and subsequent Covid Vaccinations

  • After the vaccination you will be required  to wait 15 minutes in the surgery
  • You will be given a certificate confirming that you have had the vaccination
